This notice describes how medical information about you or your child may be used and disclosed and how you can access this information. Please review it carefully.

PEDS Urgent Care is required by law to maintain the privacy and security of protected health information, also known as PHI.

Your Rights

You have the right to:

  • Request to inspect or receive a copy of your or your child’s medical record

  • Ask us to correct medical information you believe is incorrect or incomplete

  • Request confidential communications, such as being contacted at a specific phone number or address

  • Ask us to limit certain uses or disclosures of health information

  • Request an accounting of certain disclosures of health information

  • Receive a paper copy of this notice

  • Choose a personal representative, where legally appropriate, to act on your behalf

  • File a complaint if you believe your privacy rights have been violated

How We May Use and Share Health Information

We may use or share health information for treatment, payment, and healthcare operations.

For treatment: We may use or share health information with physicians, nurses, specialists, pharmacies, or other healthcare professionals involved in your child’s care.

For payment: We may use or disclose health information to bill for services and obtain payment from you, an insurance company, or another responsible party.

For healthcare operations: We may use health information to operate our practice, improve the quality of care, train staff, manage services, and contact you when necessary.

We may also use or disclose health information when permitted or required by law, including for public health and safety activities, reporting suspected abuse or neglect, health oversight activities, legal proceedings, workers’ compensation, law enforcement requests, and to prevent or reduce a serious threat to health or safety.

We will not use or disclose health information for marketing purposes, the sale of health information, or most sharing of psychotherapy notes without your written authorization, unless permitted or required by law.

Our Responsibilities

We are required by law to:

  • Maintain the privacy and security of protected health information

  • Provide you with this notice of our legal duties and privacy practices

  • Notify you if a breach occurs that may have compromised the privacy or security of your information

  • Follow the terms of this notice currently in effect

We may change the terms of this notice. Any changes will apply to all information we maintain, and the revised notice will be available in our office and on our website.
